Mechanism of Action (MOA):
- Metformin: Decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
- Sitagliptin: Inhibits the Dipeptidyl Peptidase-4 (DPP-4) enzyme, leading to increased levels of incretin hormones. This results in enhanced insulin release and reduced glucagon secretion, thereby lowering blood glucose levels.

Dosage and Administration:
- The recommended starting dose for patients not currently treated with Metformin is 50 mg Sitagliptin combined with 500 mg Metformin, administered twice daily with meals.
- For patients already on Metformin therapy, the Sitagliptin dose should be adjusted to 50 mg twice daily, with the Metformin dose tailored based on current therapy.
- The maximum daily dose should not exceed 100 mg Sitagliptin and 2000 mg Metformin.

Side Effects:
- Common: Diarrhea, upper respiratory tract infection, headache.
- Serious: Lactic acidosis (rare), anaphylactic reactions, rhabdomyolysis, acute renal failure, angioedema.

Drug Interactions:
- Before using SIPTIN-M, inform your doctor about all other medications you are taking, including prescription, over-the-counter drugs, and herbal supplements.
- Particular caution is advised when combining SIPTIN-M with other antidiabetic agents, as there is an increased risk of hypoglycemia.
- Drugs that affect renal function or are eliminated renally may interact with SIPTIN-M.
