Mechanism of Action (MOA):
Rosuvastatin works by inhibiting HMG-CoA reductase, an enzyme responsible for the synthesis of cholesterol in the liver. This inhibition leads to a reduction in hepatic cholesterol production, resulting in increased uptake of LDL cholesterol from the bloodstream and a subsequent decrease in overall blood cholesterol levels.
Dosage and Administration:
- Adults:
- The recommended starting dose is 5 mg once daily. Depending on individual response and therapeutic goals, the dose may be adjusted up to a maximum of 40 mg once daily.
- Elderly (Over 70 years):
- A starting dose of 5 mg is recommended. No dose adjustment is necessary based solely on age.
- Renal Impairment:
- For patients with an estimated glomerular filtration rate (eGFR) between 30-60 mL/min/1.73 m², initiate with 5 mg once daily (do not exceed 20 mg daily). Avoid use if eGFR is less than 30 mL/min/1.73 m².
- Administration:
- Take the tablet at any time of day, with or without food.
- Swallow the tablet whole with water.

- Common:
- Headache
- Muscle pain or weakness
- Abdominal pain
- Nausea
- Dizziness
- Serious:
- Myopathy and rhabdomyolysis
- Liver enzyme abnormalities
- Severe allergic reactions
Drug Interactions:
- Concurrent use with gemfibrozil significantly increases rosuvastatin exposure, raising the risk of myopathy/rhabdomyolysis. Combination therapy should be avoided.
- Use with caution when co-administered with atazanavir, darunavir, ezetimibe, fibrates, itraconazole, lopinavir, or tipranavir; dose adjustments may be necessary.
Precautions:
- Renal Impairment: Use with caution; dose adjustments may be necessary.
- Hepatic Impairment: Contraindicated in patients with active liver disease or unexplained persistent elevations of liver transaminases.
- Pregnancy and Lactation: Contraindicated; may cause fetal harm.
- Alcohol Consumption: Limit intake due to potential liver effects.
- Asian Ancestry: Increased systemic exposure has been observed; a starting dose of 5 mg is recommended, and the 40 mg dose is contraindicated.
